Purpose

To ensure physician coverage is available and accessible 24 hours a day, 7 days a week, including when a physician is not physically present at the clinic.

Policy

Besa Health maintains supervising and backup physician coverage for patient care, urgent clinical questions, after-hours calls, and requests from hospital emergency-room practitioners. Current clinic hours and after-hours instructions must be posted or readily available to patients and staff.

Procedure

  1. Contact Dr. Yang first for physician consultation or coverage. Use Besa Health EHR chat for routine matters and call him directly for urgent or time-sensitive matters.

  2. If Dr. Yang is unavailable, contact Dr. Yim by phone.

  3. Provide the physician with the patient's name, relevant history, current symptoms, vital signs when available, and the specific question or decision needed.

  4. Follow and document the physician's instructions in the patient's EHR record, including the date, time, physician contacted, response, disposition, and follow-up plan.

  5. If neither physician can be reached, do not delay emergency care and do not continue non-urgent care that requires physician availability. Reschedule, transfer, or refer the patient as clinically appropriate.

Emergency Situations

Call 911 or direct the patient to the nearest emergency room immediately. Do not wait for a physician callback before activating emergency services. Notify Dr. Yang or Dr. Yim after emergency care has been initiated when doing so will not delay treatment.

After Hours

The after-hours telephone message or live responder must tell patients how to obtain urgent or emergency care and the expected time for a return call. A physician or qualified designee must be available 24 hours a day, 7 days a week. Calls requiring clinical triage or screening must be returned within 30 minutes.

After Hours Response

  1. For a possible emergency, instruct the patient to call 911 or go to the nearest emergency room.

  2. For an urgent concern, call Dr. Yang. If he is unavailable, call Dr. Yim.

  3. For a routine concern, advise the patient that the appropriate team will respond on the next business day.

  4. Document all after-hours clinical advice, referrals, orders, and follow-up instructions in the Besa Health EHR.

  5. The assigned clinical team reviews the encounter on the next business day and completes any needed follow-up.
