The entire care team should fill this in with different information they learn about the Patient that might be useful for the rest of the team. This is information that we can use to create a relationship with a Patient. Example: Favorite sports team. 

Providers should add something to the notes section after their first visit. This is a sign that we know the provider connected with the Patient.
