An annual wellness exam (AWE) is the base for all care. This is what we must do when bringing on a Patient to have in our care. This involves many different things and is always subject to change. Be sure to get everything prepped for the provider.

  • We should prep for this the day before or morning of the appointment. 
  • Anytime you see an appointment reason for Annual Checkup or anything similar, it lets you know that you need to prepare using the information in this article. 

     
  • We will always need to make sure that we have the supplies from LabCorp. They have a hard time getting us the supplies. 

     
  • After the blood is drawn, it will need to be spun in the centrifuge. 


     

📘 Instructions

Be sure to follow all these steps so we don’t miss anything.  

  1. Look on the calendar page to see what appointments coming in would be an Annual Wellness Exam of some sort. 



     
  2. Print out the PHQ-9 and GAD-7

     
  3. Prepare the LabCorp bag: 

    1. Serum gel tubes x2
    2. Lavender top 
    3. Specimen bag
    4. Butterfly needle 
    5. Chuck
    6. Gauze pad
    7. Alcohol prep pad
    8. Touriquet
    9. Vaccutainer cover
    10. Ensure there are cotton balls, medical tape, etc to cover specimen collection site.  

      Bold = Needed for all labs

⚠️Be sure to review the process as there are different things we may need to do based on the specimen. For example, centrifuge. 

  1. Put everything on a steel tray with Patient first name on a sticky note so the provider knows what to pull.
